  Abstract
Introduction: Incisional hernia is the most common cause of intestinal strangulation in patients presenting with intestinal obstruction, potentially complicating with intra-abdominal hypertension syndrome and its pathophysiological consequences. Case Report: An obese woman with a strangulated incisional hernia is herein reported. We discuss the severe consequences and lethal risks to which this patient was exposed as a consequence of her hernia. The clinical course and treatment is reported, stressing the most important clinical and pathological aspects. Conclusions: This gigantic incisional hernia developed because the patient and her physicians neglected the surgery due to a diverse number of factors, the most important of which was morbid obesity and fear of postoperative complications frequently developing in such patients.
